Add/Update Patient - Location

You use the Add/Update Patient function to add or update demographic information, including the patient's name, birth date, marital status, addresses, and special handling codes. This function does not  update the registration date. You can add a patient to the system using this function, but the  patient will not be registered. You must use the Registration function.

To begin entering information for a new patient:

  1. Select Add/Update Patient from the Patient menu.
  2. Enter information about the patient for whom you want to enter demographics on the Patient Selection screen. The screen lists patients who meet the search criteria you entered.
  3. On the Patient List screen, click on the New button to add a new patient.

To begin adding demographics for an existing patient:

  1. Select Add/Update Patient from the Patient menu.
  2. Select the patient for whom you want to add demographics.The screen lists patients who meet the search criteria you entered.
  3. On the Patient List screen, select the patient you want to update.You enter information about the patient on the patient demographics screen.

To enter or update patient demographics:

  1. Enter the patient's name in the Patient Name field. Enter the patient's last name, a comma, and the first name (for example, SMITH, JANE).
  2. Enter the patient's birth date in the DOB field. Enter the date of birth in the MMDDYYYY format with leading zeros for single digit numbers (for example, 06041940 for June 4, 1940).
  3. Enter the patient's social security number in the SSN field.
  4. Select M for male, F for female, or U for unknown.
  5. Select the code for the patient's marital status in the Marital Status field. The codes for the marital status are M for married, S for single, D for divorced, P for separated, W for widowed, or U for unknown.
  6. Select the code for the patient's race in the Race field. The codes for race are A for Asian, B for Black, H for Hispanic, I for Native American,  O for Other, W for White, or U for Unknown. 
  7. Enter the patient's mailing address in the Mail Address, City, State and Zip fields.
  8. Enter the county in which the patient lives in the County field.
  9. Enter the patient's telephone number in the Telephone fields.
  10. Enter any other patient information you have.
  11. Check that the information is correct and press Enter or click OK.
